TERMS OF SERVICE
These terms govern your membership agreement with Dr. Hi Hello. We are a direct primary care practice, not an insurance company.
MEMBERSHIP AGREEMENT
By enrolling, you agree to a continuous primary care relationship outside of the traditional insurance fee-for-service model.

| Cancellation | Requires 30 days written notice. Early termination within the first 3 months incurs a fee. |
| Scope of Practice | We provide primary care services. We are not a substitute for specialist oncology, surgery, or emergency care. |
TERMS FAQ
Is this insurance?
No. This agreement does not provide comprehensive health insurance coverage. It provides only the health care services specifically described in the membership agreement.
Can you dismiss me as a patient?
Yes. We reserve the right to terminate the physician-patient relationship (with appropriate 30-day notice for continuity of care) for non-payment, abusive behavior towards staff, or non-compliance with critical clinical protocols.
